RN Liaison - Hospice-Weekend Track - Eligible for Full-Time Medical Benefits

The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as Liaisons in local hospitals (Capital Region-primarily Samaritan Hospital, Troy, NY and Ellis hospital, Schenectady NY). This position is ideal for someone who needs time during the week.

Schedule: Only Every Saturday and Sunday 8am - 8pm. No days during the week after training period

What You Will Do

  • Evaluate patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care.
  • Coordinate with the patient’s community physician and our hospice medical directors to confirm eligibility for hospice services.
  • Gather information and ensure a smooth handoff to the hospice RN admitting the patient/family to services.
  • Admit patients to Hospice in the hospital setting upon eligibility.
  • Work closely with the hospital case management department.
  • The work schedule is flexible, generally weekdays (M-F) between 8am to 4pm. Weekend opportunities may also exist from time to time.

What You Will Need

  • A current license to practice as a Registered Nurse in the State of New York
  • Associates Degree in Nursing, BSN preferred
  • 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.
  • Hospice or RN Liaison experience a plus but not required.
  • The RN must be able to communicate effectively, verbally and written
  • Valid Driver's License and Reliable vehicle
  • High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
  • Understanding of Hospice principles preferred

Enhanced rate for weekend track: $52.30/hr

Experience based pay range for NON-weekend track: $35.00-$47.75 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
